Access and Privacy

By law, you have the right to access your personal health information with limited exceptions. The Personal Health Information Act ensures that you get timely access to your medical records and also requires that we keep your information private, safe and secure.

Your personal health information is recorded information about you that we keep in our medical records, including your name, address, Personal Health Identification Number (PHIN), information about your health, health-care history, and the care that has been provided to you.

If you are wanting a copy of your personal health information you can either present in person to the Health Information Services Department or complete the following form, Request To Access Personal Health Information, and forward to:
