TeleCARE TéléSOINS Manitoba

TeleCARE TéléSOINS Manitoba is a free, confidential, telephone- or telehealth-based program that helps Manitobans self-manage their Heart Failure, Type 2 diabetes or risk of diabetes.
The TeleCARE TéléSOINS Manitoba team of registered nurses and registered dietitians are experienced in working with people living with chronic disease.
A schedule is developed for appointments, based on the client’s needs and availability. These discussions focus on working together over the telephone or telehealth to develop the knowledge, skills and support required for the client to self-manage their health. The client may also call any time during business hours to speak with a TeleCARE TéléSOINS nurse and can always reach a nurse 24/7 through the Health Links – Info Santé program, which answers calls off hours.
During appointments staff will assess any health changes or new symptoms and provide education and support to assist the individual to:
- understand their disease,
- problem solve for issues that arise and
- develop and work toward realistic health change goals to improve their health.
The goal is to empower people to independently manage their own illness as much as possible. TeleCARE TéléSOINS Manitoba staff work with the primary health-care provider/team to support the primary treatment plan for the client.
People may phone to refer themselves or can be referred by a health-care professional.
Referral forms completed by the health-care provider can be faxed to the TeleCARE TéléSOINS Manitoba program at 204-779-5645.
People are eligible for the TeleCARE TéléSOINS Manitoba program if they are age 18 or older and living anywhere in Manitoba with:
- Diagnosed Heart Failure
- Type 2 Diabetes and not pregnant
- Pre-diabetes or high risk of diabetes (history of gestational diabetes, family history of diabetes)
Clients or their care providers must be able to participate in this program by telephone and have a primary health-care provider who is willing to collaborate with the TeleCARE TéléSOINS Manitoba program staff.
